OSNA at Ashford Hospital
Aug, 28th 2009At Sysmex Life Science, we aim to build a team ethic between our own personnel and all staff involved in offering what is an important additional service for the appropriate management of patients. We believe that this team approach is vital in order to build confidence in what for many, is their first exposure to intra-operative analysis for metastatic breast cancer.
The development of this team relationship is well illustrated by the Training carried out recently with the Staff of the Cytology Department at Ashford and St. Peters NHS Trust here in the UK. The Department, working with the Breast Surgery Team led by Miss Sunita Shrotria is embarking upon a six month clinical implementation programme assessing the technical and logistical impact of One Step Nucleic acid Amplification (OSNA) for the management of patients at the Trust. Dr Behdad Shambayati, Consultant Clinical Cytologist at the Trust said "We are happy to support Miss Shrotria and her team wherever we can with the aim of introducing what looks to be a very significant step forward in the management of our patients".
The Training was carried out against the backdrop of the UK Comic Relief Charity Appeal “Red Nose Day” Since 1985, Comic Relief has inspired The Nation to laugh while raising money to help change lives. The money raised helps poor, vulnerable and disadvantaged people throughout the world and has raised over £600 million since its inception and over £60 million in 2009 alone. I would like to thank my fellow members of our “OSNA Team”, Behdad, Gillian and Francesca, for their enthusiasm both with regard to our training and the implementation of OSNA, and indeed on behalf of Comic Relief 2009, as the following evidence clearly demonstrates ...

XV Reunión de la Sección de Patología Mamaria de la Asociación Española de Cirujanos, Valencia 21 y 22 de Mayo de 2009
Jul, 24th 2009The XV meeting of the section of breast pathologists from the Spanish association of surgeons started on the 21st May with a practical session ("I Curso Práctico International de Cirugía Radioguidada Mamaria") in the Hospital La Ribera in Alzira/ Spain. More than 90 surgeons from all over Spain participated. As part of the course, Sentinel lymph node mapping by radio-guided detection was demonstrated in a live surgery which was performed in the operating theatre and transmitted via video documentation to the audience in the conference room. The examination of the Sentinel lymph node was done with OSNA in the laboratory of the Pathology Department by Dra. Laia Bernet, pathologist from Hospital Lluis Alcanyis/ Xàtiva, who has used OSNA in routine practice since late 2008. The instrument and the reagents were provided by Sysmex. The whole working process could be followed on a second screen and commentary was given by the pathologist from Alzira Dr. Rafael Cano. The result (OSNA detected a macrometastasis), representing the status of the whole lymph node, was obtained within 34 minutes. As a consequence, ALND was performed during the same operation following resection of the primary tumour.
On completion of the analysis Dra. Bernet also joined the audience to give a short presentation of the method and to be available for questions and discussion. She particularly emphasized the significance of a standardised procedure for intra-operative SLN diagnosis and the benefit of a method with high sensitivity.
This demonstration not only aroused a lot of interest by the surgeons but was also reported on TV (Channel 9).
The meeting continued in the afternoon and the following day with the remaining program. There again Dra. Bernet reported their experiences with OSNA in a plenary session, which again created a lot of interest. The poster presented by the Hospital from Xàtiva was awarded the first prize of the meeting.

St. Gallen Oncology Conference on Primary Therapy of Early Breast Cancer, 11th-14th April
Apr, 6th 2009The 11th International Conference on Primary Therapy of Early Breast Cancer was held in St. Gallen, Switzerland from 11th to 14th April. At one of the biggest meeting of breast cancer specialists Sysmex presented OSNA, a test for the detection of lymph node metastases in breast cancer, on a booth in the exhibition area.
Sysmex also held the 2nd Symposium on OSNA. Dr Christian Schem from University Hospital Kiel started with introducing the topic to the audience. Presentations by Dr Abir Khaddage from University Hospital St. Etienne, Dr Christian Schem and Dr Simonetta Buglioni from National Cancer Institute Rome followed. They explicitly explained the method and reported their experiences with OSNA. Also design and results of the French multi-centric clinical study and the German performance evaluation were presented. As the studies had shown very good results and their own experiences with the method had been very positive, all three hospitals moved into routine use within the last year.
The main advantages of the lymph node analysis with OSNA from their point of view are a high sensitivity and specificity, standardisation of sentinel node analysis and reduction of second surgeries caused by false negative intra-operative results. Generally the presenters explained that OSNA is comparable to extensive histopathological examination, with a time need of about 30 minutes. Anyway they as well put emphasis on the fact that there is no standardisation for the histopathological analysis of sentinel nodes in most European countries and that with the introduction of OSNA this can be improved. But most of all, the patient benefits from the definitive intra-operative result provided by OSNA as well as the hospital by saving patient bed days and theatre use.
To conclude the presentations Dr Schem gave an overview of the finalised studies in Europe and Japan and explained pooled data from these studies. So far over 2,300 lymph nodes were analysed, leading to a concordance of 96.5%. The overall sensitivity is 95.6% and the specificity is 96.7%. He also mentioned that so far 15 hospitals in Europe use OSNA in routine, further installations are in progress.

CHU Saint-Etienne shares their experience of innovation
Mar, 20th 2009On 12th February 2009, Professor Peoc'h, Chief of the Pathology Department and Dr Khaddage, pathologist in charge of gynecological examination at the University Hospital in Saint-Etienne (France), welcomed health professionals, surgeons and pathologists interested in the implementation of OSNA into routine use.
The purpose of the visit was to meet the different teams involved in the management of breast cancer patients undergoing surgery including the excision of sentinel lymph nodes, to report on the organisational impact for both pathologists and surgeons and to introduce intra-operative analysis using OSNA. The analysis of a sentinel node was performed in real time during the visit and the simplicity and ease of the test was clearly illustrated.
Dr Mehddi, gynecologist surgeon in the department of Obstetrics Gynecology of Professor Seffert at University Hospital in Saint-Etienne commented:
»Even if the time of analysis is slightly longer than the conventional intra-operative tests, it allows the surgeon to remove the tumour and the pathologist to concentrate on a more accurate analysis of margins, and the benefit of accurate intra-operative analysis for the patients is undeniable«
»Since the routine use of this test started in June 2008, 11 patients have avoided a recall for axillary dissection ensuring a better level of health care for the patient«, said Dr Khaddage.

The 30th Annual Meeting of the French Society of Senology and Breast Pathology (SFSPM) was held between 05th and 07th November in La Baule, France. The meeting focused on hormonal and societal factors, genetic risk and the prevention of breast cancer. More than 1,100 gynecologists, oncologists, radiologists, pathologists, epidemiologist, and biologists attended the conference.
On Wednesday 5th November the afternoon session included a symposium organized by Sysmex dedicated to the progress in the molecular analysis of sentinel nodes in breast cancer. Professor Peoc’h from University Hospital in Saint Etienne and Dr Peston from Charing Cross Hospital in London respectively each gave a presentation about the results of the French and the British multicentric evaluations of OSNA in a clinical intra-operative setting. During this session both Dr Bouteille, gynecologist at University Hospital Saint Etienne and Dr Buglioni from Regina Elena Tumor Institute in Rome, shared their experiences and opinions regarding their use of OSNA in the routine clinical diagnostic arena. “The stronger point was that in all the different centres, results were similar and the benefits expected were the same everywhere” explained Dr Bouteille. The presentations were followed by a lively and passionate discussion between the speakers and an enthusiastic audience which gave a strong indication of the importance of the subject and the clear and obvious requirement to make an accurate intra-operative diagnostic approach to patient management more widely available. The clear consensus was that the use of OSNA as a molecular diagnostic tool for the intra-operative analysis of sentinel nodes offered an important step forward in optimising the clinical management of the patient in this setting.
